Patient Related Outcome Measures and Social Determinants of Health in Chronic Disease

Social determinants of health (SDH) are the non-medical factors impacting health outcomes. SDH comprise a set of process outcomes that may help or hinder specific populations to get healthy, stay healthy and importantly to access optimal healthcare resources in their communities. The shift to value-based care seeks to incentivize prevention of illness and chronic disease through appropriate lifestyle behaviors, but we also need measures to address these SDH in order to overcome these longstanding inequities and improve outcomes. This Thematic Series considers all SDH in chronic disease – such as diabetes, COPD, kidney disease, and heart disease.
