Following publication of the article, it has come to the authors attention that there are some errors in the CPRD Aurum medcodes due to Excel rounding the longer codes. The authors have advised the error has a small impact on the numbers in Table 2 and Table 3 on pages 1030-1031 and 1033, respectively, of the published paper. The error also impacts the Supplementary Data, specifically: Supplementary Table 1(b), Supplementary Table 2 and Supplementary Figure 1(a).

The correct Tables 2 and 3 are shown below. The correct supplementary material can be found here: Supplementary Data.
