On page 4751, Ethics Approval and Informed Consent section, third sentence, the text “Protocol modifications will be approved by the medical research ethics committee of the Hospital of the Kyoto Prefectural University of Medicine (protocol reference number: R000040969)” should read “Protocol modifications will be approved by the medical research ethics committee of the Hospital of the Kyoto Prefectural University of Medicine (protocol reference number: ERB-C-1463-2)”.
