The authors have advised there are errors in Figure 3 on page 878. Figure 3A, the first set of numbers listed for the number of patients at risk below the survival graph are transposed. Figure 3B, all of the numbers listed for the number of patients at risk below the survival graph for the “Invited” and “Not Invited” category are transposed.
These errors do not affect the results of the paper.
The authors apologize for these errors.
