Noteworthy Comment: Loss of muscle mass or sarcopenia is a complex, multifactorial process which in older adults is increasingly recognized as a high-prevalence clinical entity. It is associated with a high risk for falling, reduced functionality, and increased mortality. Because there is relatively little known about actual dietary protein consumption especially in relation to mealtime the authors of this study assessed these factors while simultaneously exploring their association with appendicular skeletal muscle mass in apparently healthy older adults. Their findings led them to conclude that older adults not only fail to receive the quantity of dietary proteins recommended to prevent sarcopenia, but experience sub-consumption of proteins per mealtime with breakfast and dinner being the most compromised. The importance of these findings was the significant association between protein consumption per mealtime and muscle mass loss. In addition to dietary protein intake, the need for studying associated risk factors such as insulin resistance, cytokines, vitamin D deficiency, sedentarism was stressed as a potentially effective strategy for better opposing the maladaptive changes in body composition associated with aging.
